Sinnissippi Centers is in need of a Care Navigator in our Freeport office! Sinnissippi is expanding our services along with the Illinois CCBHC program to ensure access to coordinated and comprehensive behavioral health care and our commitment to person-centered treatment. A Care Navigator helps manage services and make a positive impact on the client's behavioral healthcare journey. The position plays a crucial role in ensuring clients are connected to the appropriate care coordination, behavioral and/or physical health services within the organization, exploring any barriers that might negatively impact engagement with these services and collaborating with internal teams to promote best outcomes. The Care Navigator may also provide screening, triage, crisis intervention, and referral to supportive services. 

Objectives:

  • Serve as a reliable intermediary to reduce socio-economic and cultural barriers between clients and healthcare institutions. 
  • Utilize Telehealth and networking systems to connect clients to providers in real-time during off-site visits and assist clients in accessing digital health-related services. 
  • Maintain records, participate in outreach events, and stay informed about community resources. 
  • The Navigator acts as an advocate for member's needs by identifying and addressing gaps in services and support.
  • Engages clients in inpatient settings to facilitate post-crisis planning, assist with transitions from different treatment settings and navigating multiple systems.
  • Implement, coordinate, and monitor strategies for clients to improve health and quality of life outcomes.
  • Maintain professional responsibility and respond appropriately to personal stressors that impact ability to perform job duties and recognize crisis situations or risks to the member's safety and respond appropriately.
  • Assists clients in developing problem-solving skills so that they can respond to challenges that may arise in their recovery process.

Qualifications:

  • Bachelor's degree in a human services field.
  • Reliable vehicle with the ability to travel within the community regularly (mileage reimbursed).
  • Ability to make decisions that require significant analysis of solutions, and quick, original, and independent thinking.
  • Knowledge of local mental health, substance use, and community systems is beneficial.
  • Strong interpersonal and organizational skills and effective verbal and written communication skills.
  • Ability to summarize and document findings and maintain complete and accurate records.
  • Must be able to work effectively, independently and in a team, and prioritize in a fast-paced environment to meet the demands of the organization.

Salary: $51,405 - $53,975 (MHP/Bachelor's level); $59,116 - $62,071 (QMHP/Master's level)
