SUPR Intake Coordinator

Sinnissippi Centers seeks to hire a SUPR (Substance Abuse Prevention & Recovery) Intake Coordinator to ensure access to coordinated and comprehensive substance abuse and behavioral health care. This position will provide intake and care coordination to help people navigate behavioral health care, substance abuse treatment, and the other systems they are involved in.

The qualified candidate will be working with youth, families, and adults at Sinnissippi's Dixon/Route 2 location providing effective treatment for individuals and families with substance abuse and behavioral health needs.

This role provides expert psychosocial assessment, brief targeted intervention, and individual therapy for social, emotional and environmental issues that impact client/family ability to optimally benefit from care. The role also provides education and or consultation to patients and their families.

This position provides a great opportunity for a self-directed mental health clinician to make a significant contribution to the larger care team. Working as an independent health care provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to clients while working collaboratively within a multidisciplinary health team.

We are seeking an Intake Coordinator to:

  • Provide Psychosocial assessments to youth and adults.
  • Collaborate with other staff, clinicians and medical professionals in a fast-paced environment.
  • Provide brief evidence-based interventions.
  • Provide short, but intense follow-up.
  • Connect clients to resources.
  • Use outcome-based models to track client improvement.

Work Location: In person at our Route 2 office

Salary: $51,405 - $53,975 (MHP/Bachelor's level); $59,116 - $62,071 (QMHP/Master's level)

Requirements

  • Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Ability to effectively engage patients in a therapeutic relationship, when appropriate.
  • Experience with screening, assessing, and carrying out treatment plans for common mental health disorders.
  • Working knowledge of differential diagnosis of common mental health disorders.
  • Working knowledge of evidence-based psychosocial treatments for common mental health disorders.
  • Basic knowledge of psychopharmacology for common mental health disorders.
  • Familiarity with social/human service practices, confidentiality, and risk/malpractice issues within health care settings.
  • Bachelor's degree and/or nursing license required; and previous clinical experience.
