What you will do face to face with patients:
· Greet patients and escort them to the primary care wing of our office at 325 IL Rt 2 in Dixon
· Obtain height, weight and vital signs
· Assist with collection of chief complaint, health history and screening needs
· Enter active medications, allergies, pharmacy information, etc.
· Assess tolerance to new or changed medications
· Provide medication samples
· Assess need for immunizations due and alert provider of specific needs
· Collect labs i.e. throat swabs, urine dips, covid swabs, influenza swabs, blood draws, etc.
· Obtain accu-checks
· Perform injections
· Perform wound care or dressing changes
· Perform nebulizer treatments
· Provide written visit/diagnosis education
· Schedule follow ups
· Fax orders to labs
· Assist with immediate medication orders for patients with acute needs such as high blood pressure or diabetes
· Respond to medical emergencies
What you will do on behalf of patients:
· Scheduling appointments
· Process and fax referrals
· Follow up with patients to ensure labs are getting drawn
· Obtain records from other providers
· Triage phone calls
· Communicate with other services such as transportation, residential treatment providers, recovery homes, etc.
· Communicate with pharmacies and labs
· Assist patients with obtaining medication from Sinnissippi's onsite pharmacy, Genoa
· Assist patients with compiling an accurate and complete medication list
Some administrative functions you can expect to complete:
· Room prep between patients
· Chart prep for future patients
· Supply inventory and stocking/restocking
· Preparing and sending letters of missed appointments
· Assist in the creation of processes for workflow
As this is a newly created role, it is expected that there will be other reasonable duties not listed that will become expectations.
Successful candidates will have:
· Strong collaboration and communication skills with the ability to work in cross-functional teams
· Self-motivation and an interest in being involved in new ventures
· Ability to make good decisions for situations that might not be standard practice
· Good computer skills and the ability to navigate electronic medical record software
Who you are (basic qualifications):
· Licensed as an LPN or RN in the State of Illinois
· 3-5 years of experience in outpatient healthcare settings
· Stable work history
· Interest or experience with patients with a behavioral health or substance use diagnosis

Base salary starts at $62,071/year. This is a salaried position. To meet patient needs the schedule for this position is Monday-Friday 8:00 a.m. - 5:00 p.m. on site at 325 IL Route 2, Dixon.
