Sinnissippi Centers (operating as a Care Coordination & Support Organization--CCSO) is hiring a new Care Coordinator to establish access to high-quality home and community-based services for children with significant behavioral health challenges. This System of Care improvement initiative (titled Pathway to Success) will focus on supporting children's success at home, in school, and in their communities in the Whiteside, Lee, Bureau, and Henry county areas. This position is home based and would focus on Whiteside, Lee, and Bureau Counties.
JOB DUTIES:
Salary: $51,405 - $59,758
